Tamil Nadu has consistently demonstrated a strong commitment to maternal and child healthcare and has achieved remarkable progress in "Ensuring Healthy Mothers and Healthy Newborns" through sustained investments in public health infrastructure, skilled human resources, and comprehensive maternal and child welfare programmes,

The State has accorded high priority to maternal healthcare and has implemented several initiatives aimed at providing financial assistance to pregnant women, free delivery and newborn care services, emergency obstetric care, nutritional support, birth preparedness, and digital tracking of beneficiaries. These initiatives supported by a strong network of healthcare institutions and skilled healthcare personnel, have contributed significantly to improving maternal and neonatal health outcomes in the State.
3. Every year, the State records approximately 7.8 lakh deliveries, with institutional deliveries constituting 99.9% of all births as per NFHS-6. Among this, Government health facilities contribute nearly 4.2 lakh deliveries (53%) which reflect the central role of the public health system in providing maternal and newborn healthcare services. Among the Government institutional deliveries, 90% are conducted in 130 Comprehensive Emergency Obstetric and New-born Care (CEMONC) centres.
4. The accessibility and affordability of these services offered at Government Institutions have strengthened financial protection for families and contributed significantly to improved maternal and neonatal health outcomes. This has been substantiated by National Sample Survey (NSSO 80th Round) which states that the average out-of-pocket expenditure per delivery in Government hospitals is only Rs. 1,364/-, compared to Rs.63,473/- in private hospitals. Government hospitals serve as the primary safeguard against major out-of-pocket healthcare expenditure and the high costs associated with diagnostics and referrals. 5. Recognizing the welfare and dignity of pregnant mothers and with a view to further strengthening confidence in public healthcare services, the Government of Tamil Nadu have decided to provide "one-gram Gold Ring" to newborns delivered in Government Institutions as a token of appreciation and remembrance, commemorating the birth of the child and celebrating the joy and significance of motherhood through the Thaimaaman Thanga Mothiram Thittam (momocör gilis மோதிரம் திட்டம்).
6. As per the cultural idea of Thaimaaman Seer in Tamil Tradition, the Maternal Uncle (Thaimaaman) presents ceremonial gifts (seer) - customarily including Gold - to welcome the newborn and bless the family. Through this scheme, the Government assumes the role of the maaman for every child born in its institutions, presenting a Gold Ring as an enduring token of welcome.
7. Gold carries profound cultural and emotional resonance in Tamil Nadu. Gifting Gold to a newborn celebrates the baby's arrival, honours the mother's noble journey and transforms public health delivery into a source of immense community pride. Also, this token remains an enduring asset, generating lasting institutional goodwill that spreads via word of mouth.
8. This culturally meaningful initiative seeks to celebrate motherhood, honour the mother's journey, reinforce confidence in public healthcare services, and strengthen the bond between people and public institutions. The initiative is intended to further strengthen the State's commitment to maternal and child health by promoting equitable access to quality healthcare and ensuring that every mother and new-born receives safe, affordable, and compassionate care through the public health system.
9. The modalities of the Thaimaaman Thanga Mothiram Thittam (mumGÅT sha Congru ALL) are detailed below: -
a. Goals and Objectives 0 (ii) Elevate public healthcare settings from a baseline necessity to a preferred destination of pride and celebration, with dignity at birth. Reclaiming the Government institutional deliveries by offering a high-value, permanent tangible benefit that complements the existing maternity cash benefits and maternal kits.
(iii) Erase socio-economic bias surrounding public health facility deliveries, recognizing that every mother's journey is noble and every child is a treasure.
b. Target Beneficiaries with Eligible criteria (0) (ii) Resident of Tamil Nadu is eligible for the scheme.
All pregnant mothers delivering at Government institutions. (iii) Mother should be PICME registered with RCH ID. (iv) The mothers having newborns are eligible under the scheme.
-3- (v) Beneficiaries must submit residential proof through any one of the following documents:
(i) Ration Card; (ii) Voter Identity Card; (iii) Aadhar Card; (iv) ID Card issued by the Labour Welfare Boards; (v) "Proof of Address Card" issued by Department of Posts; (or) (vi) Nativity Certificate.
(vi) The scheme extends coverage to all newborns, regardless of birth order. Mothers with Higher Order Births (HOB) are eligible for benefits without restriction.
(vii) The distribution of Gold Rings under this scheme shall be conducted without any gender-based discrimination. Benefits are provided irrespective of the gender of the new born child.
c. Program Methodology & Institutional Framework B. Out of approximately 4.42 lakh annual newborns recorded across Tamil Nadu's Government medical facilities, the institutional distribution of deliveries is structured as follows: 1. Medical College Hospitals: 54%
II. District & Sub-District Hospitals: 37%
III. Primary Health Centres (PHCs): 9%.
To ensure logistical efficiency while maintaining absolute coverage, a specialized Hub-and-Spoke Model will be implemented in the Government institutions.
The Hub Network (94 High-Volume Centres)
A group of 94 high-load Comprehensive Emergency Obstetric and Newborn Care (CEMONC) centres, that covers 82% of the Government deliveries, will serve as the primary logistical Hubs. (Annexure-A)
The Dean of the Medical College Hospital wherever the Medical College Hospital is available and Chief Medical Officer/ Medical Superintendent of the District Head Quarters Hospital / Sub-District Hospital will be the designated custodians.
Consignments will be secured inside dedicated lockers under 24/7 CCTV surveillance, backed by a resilient two-tiered power backup system.
The Spoke Facilities
The remaining 18% of deliveries are spread across smaller facilities. (ii) For these spoke facilities, 94 Hubs will act as the consolidated distribution nodes.
0. Management & Chain of Custody (i) The procurement of "one-gram Gold Ring" will be through the Tamil Nadu Medical Services Corporation (TNMSC).
(ii) The TNMSC will procure and supply the "one-gram Gold Ring" based on the yearly indent given by the Director of Public Health and Preventive Medicine.
(iii) TNMSC empanelled vendor shall supply directly to the 94 Hospitals designated as Hub. To eliminate multi-layered transit risks, shipments will travel directly from the TNMSC empanelled vendor straight to the respective facility. This transit pipeline is fully insured and utilizes tamper-evident sealed packaging. (iv) A State Project/Program Management Unit (SPMU) or State PMU, a dedicated administrative body, shall be set up to implement, monitor, and manage the Thaimaaman Thanga Mothiram Thittam (தாய்மாமன் தங்க மோதிரம் திட்டம்) scheme. A designated Logistics Manager may be placed in each district to handle cross-facility coordination and mandatory monthly stock verifications at the district level.
(v) (vi) A dedicated monitoring and evaluation will be done by a nodal person at the State level, who will oversee State-wide compliance and impact analytics.

d. Technical Specifications & Sourcing To guarantee absolute quality and fiscal accountability, all items must adhere to rigorous public procurement and standard technical criteria. (ii) Each Gold Ring will feature a fixed weight of one gram, complete with an official Bureau of Indian Standards (BIS) hallmark and the embossed Government of Tamil Nadu Logo.
(iii) Every individual unit will be uniquely serialized and protected by a tamper-proof seal with branding. Sourcing will be handled transparently through the TNMSC via an open competitive bidding process as per the Tamil Nadu Transparency in Tenders Act, 1998 and Rules.
(iv) The rate contract will be linked directly to daily benchmarks set by the India Bullion and Jewellers Association (IBJA) and the Multi Commodity Exchange (MCX) to ensure competitive, market-aligned pricing.
e. Rigorous Verification Workflow
To ensure absolute transparent distribution, a multi-layered verification process will be conducted under active CCTV monitoring: (i)
Verification of the official hospital discharge summary alongside any one of the identity proof mentioned: (i) Ration Card; (ii) Voter Identity Card;
(iii) Aadhar Card;
(iv) ID Card issued by the Labour Welfare Boards;
(v) "Proof of Address Card" issued by Department of Posts; (or) (vi) Nativity Certificate.
(ii) Real-time deduplication will be enforced via a dual Facial Recognition System (FRS) and Iris scan.

At the time of discharge, eligibility will be authenticated through the delivery records and the mother's unique RCH ID linked to Aadhaar/PICME. (iv) Immediate digital receipt entry and biometric acknowledgment will be logged directly into the PICME Portal, while inventory entry and buffer maintenance will be synchronized seamlessly. (v) PICME portal will be enabled accordingly with Facial Recognition System for the Beneficiaries.
